Throughout the course of the Inquiry, we have been persistent in our efforts to persuade the Chair to hear directly from you, the bereaved families. We have made this argument for every module, and we will continue to make this argument for Module 5, which will examine the government’s procurement processes during the pandemic.
As you will see the survey is split into three parts:
- The questions in part one are designed to help us learn more about your connection with the key issues in Module 5,
- The questions in part two are designed to provide you with the opportunity to share the story of your loved one,
- The questions in the third section allows us to gauge your willingness to be a witness in Module 5.

If you are interested in potentially giving evidence at the Covid Inquiry as a Module 5 witness, please complete this webform no later than 6th September 2024. Please note, it will be a matter for the Chair to decide who, if any, of our clients then get called to give evidence at the Inquiry.
