Throughout the course of the Inquiry, we have been persistent in our efforts to persuade the Chair to hear directly from you, the bereaved families. We have made this argument for every module, and we continue to make this argument for Module 6, which will examine the Care Sector.

To that end, we are appreciative that you have taken the time to answer this survey. The survey itself is split into three parts, the first part will ask for detail on how your loved one contracted covid; the second is designed to help us learn more about your connection with the key issues in Module 6; and the third is your opportunity to share more details from the story of you and your loved one. Please do not feel the need to answer all of the questions, not all may be relevant to your experience or that of your loved one.
